Educational revision for paediatric trainees. Emergency treatment must follow current local neonatal/paediatric and metabolic protocols with specialist support. Verify all drug doses, concentrations and infusion calculations at the point of care. Case values are illustrative.

Start with four questions

• Is the child unstable? Stabilise and call for help.
• What triggered deterioration: fasting, illness or a new feed?
• What are glucose, ketones, gas/anion gap, lactate and ammonia telling me?
• Which samples can I collect without delaying treatment?
